The module evaluation survey will be shown to all students in the Moodle dashboard. If you want to make the survey available within your module page please see instructions below on how to set this up. Please note, you will need to have an administrator or editing teacher role in Moodle to add the survey.

There are 2 ways to add the survey to your module and the blocks version is the most preferred method as it will have maximum visibility for the students taking the survey.

Adding survey as a block

  1. Go to your Moodle module page and toggle Edit mode on the top right of the page.
  2. From the right-column, click ‘Add a block’ and choose ‘Evaluations (Evasys)’ to add the block to your page.
  3. Next, move the block to the top-centre of the page for maximum visibility.

Adding survey as a LTI

  1. Go to your Moodle module page and toggle Edit mode on the top right of the page.
  2. From the section you wish to add the survey, click Add an activity or resource and choose External
  3. Next, provide an activity name and from the preconfigured tool drop-down list choose Evaluation survey
  4. Click Save and display
